These forms are needed upon registering Member Forms Girl Health & Medical History Form - All girl members of AHG must complete this form prior to attending any Troop meetings or activities. Parent/Guardian Permission Slip - Must be completed and returned to Troop Leader prior to girl participation in any AHG Troop activity. High Adventure Activity Medical Form - All participants taking part in a "Restricted High Adventure Activity" must complete this form and return to Troop Leadership prior to activity. Restricted High Adventure Activities include, but are not limited to: boating, canoeing, sailing, cave exploring, downhill skiing, flying, horseback riding, whitewater kayaking, whitewater rafting, rock climbing, rappelling, and challenge courses.
